IMPORTANT NOTICE: Upon any update to the Prior Approval Rate Application, Prior Approval Rate Template, and/or Standard Exhibits Template, filers will have a three-week grace period to transition to using the most current versions. Filings received after the three-week grace period that contain prior versions of the Prior Approval Rate Application, Prior Approval Rate Template, and/or Standard Exhibits Template will be rejected by Intake.
