Application for Appointment: Advisory Boards & Committees

Application Instructions:

Thank you for your interest in being considered for an appointment by the Insurance Commissioner.

  1. Please read the Incompatible Activities page for relevant information for all appointments.
  2. If you have submitted your application in a previous administration or were appointed under a previous administration and wish to reapply, you must fill out the current application.
  3. The application is a PDF document with fillable form fields. If you do not have software which supports this format, you may select the following link to download a free reader which can open this document: Adobe Reader.
  4. Before filling out all form fields, first download the application to your computer. Re-open it and begin filling out each field. If a particular question does not apply to you, please type "not applicable" or N/A rather than leave the space blank.
  5. Please save your completed application to your computer for your records.
  6. Email completed applications along with your current resume or bio to CDIBoards@insurance.ca.gov.
