Thank you for your interest in being considered for an appointment by the Insurance Commissioner.
- Please read the Incompatible Activities page for relevant information for all appointments.
- If you have submitted your application in a previous administration or were appointed under a previous administration and wish to reapply, you must fill out the current application.
- The application is a PDF document with fill-able form fields. If you do not have software which supports this format, download the Adobe Acrobat Reader by visiting our Free Reader and Apps page.
- Before filling out all form fields, first download the application to your computer. Re-open it and begin filling out each field. If a particular question does not apply to you, please type "not applicable" or N/A rather than leave the space blank.
- Please save your completed application to your computer for your records.
- Email completed applications along with your current resume or bio to CDIBoards@insurance.ca.gov.
