Accident and Health Agent List
The Accident and Health Agent List contains all active licensed accident and health agent individuals and/or agencies. This list contains approximately 225,000 records for individuals that hold an accident and health agent license. This list also provides the names of those individuals that may hold a second license qualification (e.g. a life-only agent license).
Order Form: Qualification Data Extract Order Form LIC DE 4. It should be noted that requests will be processed only upon receipt of the completed form and a check for the appropriate amount.
