The last page of the FLASH application service is the Checklist Page. This page is very important. Listed below are the reasons why the Checklist is important: *If you are currently licensed as a Property Broker-Agent and Casualty Broker-Agent and desire to change your license to a Personal Lines Brokers-Agent, you will need to follow the instructions for mailing an application to the CDI.
- The Checklist is the proof that the applicant filed an application for an insurance license with the California Department of Insurance (CDI).
- If the Checklist indicates there are required documents needed, the Checklist and the documents are to be mailed to CDI at the following address:
