1. Acceptance of Terms of Use for GCMS

1.1  You must read these California Department of Insurance (CDI) GCMS terms of use carefully. CDI provides this website database to You, a County employee whose County is a CDI Fraud Grant recipient or prospective fraud grant recipient. If You do not agree to all of these terms, do not use GCMS.

1.2 GCMS includes confidential information which You agree to keep confidential.

2. Your Obligations and Conduct

2.1 In consideration of Your use of the Website, You agree to: (a) Keep CDI updated if you are no longer assigned to the grant program(s); (b) Maintain the security of Your access and sign in codes; (c) Prohibit sharing of user-ids/access codes; (d) Provide accurate, current, and complete information in data-entry fields; and (e) Keep case data accurate, current and complete for each reporting deadline.

2.2 You agree to immediately notify CDI if you suspect your computer a contains any software viruses or any other computer code, files or programs designed to interrupt, destroy or limit the functionality of any computer software or hardware or telecommunications equipment.

2.3 You attest to the accuracy of the reported information, and declare, under penalty of perjury, pursuant to the laws of the State of California, that the data is true and correct.
