Collection and Use of Personal Information
Divisions 1 and 5 of the California Insurance Code and related statutes and regulations, give the California Department of Insurance (CDI) and the Licensing Services Division the authority to license insurance producers, bail agents, and adjusters. Information is collected subject to limitations contained in the Information Practices Act of 1977, SAM 5300, et seq., SIMM 5305, et seq., and other applicable state and federal laws.
Providing Personal Information is Voluntary
You do not have to provide the personal information requested. However, if you do not wish to provide us the requested information, we may not be able to provide a response to address your specific needs. When providing information or documents, please do not include unrequested personal information, such as Social Security Numbers, Driver's License Numbers, unnecessary health-related information, and credit card or financial information.
Possible Disclosure of Personal Information
We may need to share your information with other government agencies as required by law.

Unless requested by the Department, please do not include any sensitive personal information in your communications with us, including any: Social Security numbers; dates of birth; driver’s license numbers; account log-ins, financial accounts, debit or credit card numbers with any required security codes, passwords, or credentials allowing access to an account; precise geolocation; contents of mail, email, or text messages; genetic data; biometric information; information concerning your health, sex life, or sexual orientation; or information about your racial or ethnic origin, religious or philosophical beliefs, or union membership.
