Mail-In Application

New Applicants*

To use the online examination scheduling service, the applicant must complete the current Individual Application, Form LIC 441-9 and complete Item 27 "Do you wish to schedule your examination online." By answering "yes" to this question and furnishing the CDI with an e-mail address on the application (Item 20), the CDI staff will inform the applicant when the Online Examination Scheduling service is available for the applicant's use. This will allow the applicant to select the location, date, and time that is convenient for them.

Before Using the Online Examination Scheduling Service

Prior to using the Online Examination Scheduling service, applicants should check the CDI's Producer Licensing Bureau's (PLB) Weekly Processing Status Report. The Weekly Processing Status Report Web page provides a license applicant with the cashier date that the PLB is processing applications. This page is updated every Monday (except Holidays).

  • If the application has been cashiered on or after the date shown in the "Individual California Resident" column, the CDI staff will soon send the applicant an e-mail notification that the Online Examination Scheduling service is available to the applicant.
  • If the application has a cashier date that is pending processing, please monitor the "Individual California Resident" column of the Weekly Processing Status Report for the cashier date of the application. Soon after the cashier date is shown, the CDI staff will contact the applicant, via e-mail, notifying he or she that they may schedule their examination online.
