Column A: Policy Form Number.
Column B: Product Name.
Column C: Open or Closed?
Column D: Enrollment
Column E: Insured months in each policy form
Column F: Experience period on which rates are based
Column G: Period for which rates are to be effective
Column H: Total premium earned during the experience period on which the rates are based
Column I: Total dollar amount of claims incurred during the experience period on which the rates are based
Column J: Average rate increase (weighted average)
Column K: Comments
