Tax Forms, Instructions & Information

The tax forms, instructions, and information are available for current and prior year(s).  To access any year, select from the links below.

Filing options due to COVID-19 pandemic: During the period of the public health emergency, the 2020 annual tax return may be filed electronically via e-mail submission to EFT@insurance.ca.gov. The required paper original of a properly executed document must be submitted within 90 days of the lifting of the "shelter-in-place" order in California.
