Business Entity Application

You have attempted to access the Business Entity Application for Insurance License, form LIC 441-11. Pursuant to Section 1655 of the California Insurance Code (CIC), use of this form has been restricted. Specifically, this section states, in part, that no application for a license shall be deemed filed unless the document has been submitted by a means of electronic service approved by the commissioner. Most business entity insurance producer applicants must submit their application for license to the California Department of Insurance (CDI) electronically. This requirement applies to business entities applying for the following license types:

  • Life-Only Agent
  • Accident and Health Agent
  • Fire and Casualty Broker-Agent
  • Limited Lines Automobile Insurance Agent

If you are filing an application for one of the above listed licenses you must use CDI's Business Entity Online Application Service. If you are filing for a license that is not listed above, you may proceed to the Business Entity Application for Insurance License, form LIC 441-11.

Business entities may, for good cause shown, request an exemption to the electronic filing requirements. Such requests may be made by contacting the Business Entity Inquiry line at (916) 492-3069. These requests must be made prior to the filing of a paper Business Entity Application.
