How do I get licensed as a business entity?
CDI provides a Business Entity Online Application Service for business entity's applying for an insurance agency license. An online or paper application cannot be submitted to CDI unless the applicant's name is approved and reserved. How do I start the online application process?
Before you can submit your application, please select the following link to register for a new Business Entity Services (BES) account: BES Account
After setting up the BES account, open the business entity online application and insert the confirmation number assigned to your approved name in the space provided. This confirmation number will automatically upload the approved name on the business entity application. The application can be electronically submitted when completed in full. Please make sure all required documents are submitted or there will be a delay in the issuance of the license. All checklist, which are located on last page of the application, items must be completed prior to the license being issued.

What are the procedures for a Business Entity to merge or acquire another Business Entity?
Merger: CDI requires a letter signed by an officer, member, manager or all partners informing CDI of a merger and completion of the Business Entity Disclosure form, LIC BED-1, to inform CDI of any changes in officers, managers, members, partners and stockholders.
Acquisition: CDI requires a letter signed by an officer, member, manager or all partners informing CDI of an acquisition and completion of the Business Entity Disclosure form, LIC BED-1, to inform CDI of any changes in officers, managers, members, partners and stockholders.
