Business Entity Endorsement Termination

You have attempted to access the Business Entity Endorsement Termination, form LIC 411-8T. Pursuant to Section 1661 of the California Insurance Code (CIC), use of this form has been restricted. Specifically, this section states, in part, that when a business entity wants to change, remove, or add to the natural persons who are to transact insurance under authority of its license, it shall file a form prescribed by the commissioner, and that form must be submitted by a means of electronic service approved by the commissioner. Most business entities must submit Business Entity Endorsement Termination forms to the California Department of Insurance (CDI) electronically. This requirement applies to business entities submitting endorsement terminations of natural persons for the following license types:

  • Life-Only Agent
  • Accident and Health Agent
  • Fire and Casualty Broker-Agent
  • Property Broker-Agent
  • Casualty Broker-Agent
  • Surplus Line Broker
  • Special Lines Surplus Line Broker

If you are filing an endorsement termination for one of the above listed licenses you must use CDI's Business Entity Endorsement and Termination Online Service. If you are filing an endorsement termination for a license type that is not listed above, you may proceed to the Business Entity Endorsement Termination, form LIC 411-8T.

Business entities may, for good cause shown, request an exemption to the electronic filing requirements. Such requests may be made by contacting the Licensing Hotline (800) at 967-9331. These requests must be made prior to the filing of a paper Business Entity Endorsement form.
